Mental Health Assessment Centre/ Community Mental Health Teams

·         This service is able to accept routine referrals from Monday 4th May

·         The majority of clinical contacts (including new assessments) will now take place remotely through telephone and video consultation

·         The Referral for Advice function on CCG will remain in place

It is anticipated that this will be used for medication queries and as a means to discuss patients previously open to mental health services to establish if the threshold for re-referral has been met

·         No further changes have been made to urgent provision


Unscheduled Care

·         The Unscheduled Care team continues to provide a 24 hour service to the Emergency Department (ED)

·         To reduce footfall through the ED, emergency/ same day GP referrals are being assessed on the Old House Site (9am-5pm)

·         Telephone or video consultation (GP and patient) and justification of urgency is required prior to referral

·         Note that face to face consultation (GP and patient) is no longer a requirement

·         Once the referral has been accepted by mental health, the patient will be contacted by telephone and a remote consultation will be carried out

 

·         Out of hours emergency referrals will be triaged by the Unscheduled Care Team and allocated an appointment at Old See House on the following day

·         When necessary (based on clinical need / risk) assessment will be arranged on the RVH site on the day of referral

·         Should there be disagreement between the referrer and triager regarding the timing of the assessment (ie same day or next day), a face to face consultation will be requested of the GP

It is anticipated that this will apply to a very small number of patients

 

·         For all emergency referrals, the referrer will be asked for a contact number so that feedback can be provided immediately after the consultation by the assessor
